We look forward to seeing you! To ensure everyone's safety, please take note of the following:

  • The parking lot will be unavailable to cars. Please park on the street and walk over. You are welcome to drop off passengers at the entrance of our driveway.
  • Please be patient as we check you in, get you screened, and usher you to an assigned area to eat. 
  • Everyone is required to wear a mask except when in their assigned spot eating. 
  • Drop-ins are not allowed. We don't like having to turn people away - please only show up if you registered in advance.
  • In order to complete this registration please be sure to sign the waiver below.

PSC is committed to member, visitor, and staff safety and health during this troubled period. As we slowly reopen, we ask for your participation in supporting this commitment.   

Because your status and ours may change over time, we ask that you submit a waiver for each event you plan to attend in person. Please itemize below any children who will attend with you and to whom this waiver applies. Please note that other household adults will need to fill out their own form.

The Covid-19 virus is a worldwide pandemic. As a result, federal, state and local governments and agencies recommend social distancing and have restricted in-person gatherings. PSC is  doing everything possible to be compliant with all regulations and ensure your safety. Please review PSC’s COVID-19 Health and Safety Plan (the “Plan”). Congregants and guests are expected to read and comply with the measures and restrictions described in the Plan. Nonetheless, PSC cannot guarantee that any or all of the measures PSC is taking will protect you from contracting the COVID-19 virus at a PSC in-person service, event or program (collectively, “PSC Programs”). 

By participating in PSC Programs you understand, acknowledge, and agree, on behalf of yourself and persons for whom you are legally responsible, that attendance at any PSC Program involves risks and dangers which include, without limitation, the potential for injury, sickness and disease from COVID-19.  By your attendance, you willingly assume such health risk, and assume full responsibility before, during and after your participation in the PSC Program.  

In consideration of your physical attendance at a PSC Program, you hereby release, waive, and agree not to sue PSC, its employees, agents, representatives, volunteers, and other participants (the “Released Parties”) with respect to any liability, claim(s), demand(s), cause(s) of action, damage(s), loss or expense, including court costs and reasonable attorney’s fees of any kind (“Liability”) which may result from, or relate to your participation in the PSC Program. You understand and agree that this release and waiver includes any claims based on the actions, omissions, or negligence of PSC or Released Parties, whether a Covid-19 infection occurs before, during or after a PSC Program. 
YOU HAVE READ THIS AGREEMENT CAREFULLY, UNDERSTAND ITS TERMS AND CONDITIONS, AGREE TO ABIDE BY THE POLICIES CONTAINED IN IT AND IN PSC’S COVID-19 HEALTH AND SAFETY PLAN. YOU ACKNOWLEDGE THAT YOU WILL BE GIVING UP SUBSTANTIAL LEGAL RIGHTS BY SIGNING THIS AGREEMENT (INCLUDING THE RIGHTS OF YOUR REPRESENTATIVES) AND INTEND FOR YOUR SIGNATURE TO SERVE AS CONFIRMATION OF YOUR COMPLETE AND UNCONDITIONAL ACCEPTANCE OF THE TERMS, CONDITIONS AND PROVISIONS OF THIS AGREEMENT. 
Please note that the waiver applies to all names equally; you cannot have some who agree and some who don't agree
Ex: Whose Bar or Bat Mitzvah, which committee meeting, etc.
