If you are seeing "Form not available", then you have not completed the two prerequisites required in order to register for services:
Submitted your Flexible Commitment Form
Submitted your Covid-19 Proof of Vaccine to be "PSCleared" to attend services.
After doing both, please give us 24 hours to manually review and PSClear you to register!
